Go to the Optoutprescreen.com website and fill out the required form and your name will be removed from their pre-screened mailing list. This website allows you to choose to opt out for 5 years or permanently. To opt out permanently you will need to send in a signed Permanent Opt-Out Election form.
5Wait 30 days to see if the pre-screened offers stop. Call the phone number above if there is no change in your situation.
